A 17-year-old male student came to the school infirmary because of fever and lightheadedness. The nurse could not detect a blood pressure and called an ambulance.

Physical examination in the emergency department discloses that the patient has several petechiae on his arms and legs. Blood pressure is 81/55 mm Hg after administration of 1 liter of intravenous saline. He is awake and communicative but has difficulty focusing his attention. His neck is supple.
