Yur scores r very nice but the red-flags in yur CV r yur multiple attempts on yur Step-2CS exam (>1) n the number of years elapsed since yur graduation (>6).
The good thing is that u have a continuous clinical experience alongwith a PG in medicine from yur home country and many observerships from the US alongwith US LORs which will definitely help.
Wen will u be getting yur Green Card?If that happens before u apply to the programs on September 1st then that wud really boost yur chances up.Even if that happens during the remaining application cycle then that too will help.
U shud apply meticulously n only to those programs whose criteria u match.I don't think u will match the criteria of 200 IM programs if u don't have a Green Card.
Neurology programs look for an applicant's level of interest but as far as I think u don't have any experience in Neurolgy so it will be difficult for u to get Interviews in Neurolgy.
U can use FM as a back-up.

Best Wishes!

Then what other specialties should I apply to in which I have a fair chance.

Psych will also need some experience in Psych.
Do u have any interest in Paeds or Med-Paeds?Though Med-Paeds is popular amongst American Gradutes but u try yur luck in them.
U can even try Neuro.
Score high on Step-3.

Best Wishes!
