U can apply to fellowship programs in the US after having a PG degree from India or anywhere else. The problem is that one has to compete with those who ahve done residency training from US itself. It is simple common sense that they will be preferably taken into such fellowship programs.
Medical Oncology in the US is a competitive fellowship so this fact holds even more true.U need to pass yur USMLE Steps even for getting into a fellowship program.
If u can get into an IM residency program in the US then u can pursue yur fellowship of choice after comleting yur residency training.
Nevertheless, u can still give it a try.
The problem u may face of matching into a residency program r yur years since graduation. But yur IM residency training back home will be helpfull.U need to get high USMLE scores and pass all the steps on first attempt only.U can also do some observerships in the US to get some US LORs.

Good Luck!
