About Name Discrepancy

5 days back,in order to apply for the USMLE, I registered myself in the ECFMG website and gave my gmail ID as my contact ID.
The next day when i was checking mails,i found out a mail in my yahoo mailbox.when opened,i saw only user ID but no password.I read the FAQs given there.I learnt that it would be in the spam/junk folder.
so on searching for that to my surprise i found 2 mails in my past mails(unread) which are already there since very long time last april(13months back) (one containing user ID ,other containing password)
As i planned to apply for USMLE only after my final exams(now),i really dont know much about registering in ECFMG website to apply for USMLE.I mean i did not do it.I am sure i did not apply before.And in the month of april last year i was preparing for my 3rd year final exams.
so using that password and user id that the ECFMG people provided,i logged in the website and i was shocked to see that all the information about me is wrong
name is Tirunagari vamshidhar instead of vamshidhar tirunagari (i mean the surname is in the place of name and viseversa)
adress is wrong
phone number is wrong
email id is yahoo id(i never use it actually.for namesake its there created)
I then i tried registering fresh but it did not work.Now i have to suffer for the thing done by some fool.
when searched for help, i found that
there is an option of editing all these information in the oasis,except my name.I edited all the other information.
regarding my name,there is a process.I have to submit 2 legal documents(passport and birth certificate) supporting the change in name nd explain the reason for that change.
If so the legal documents are,passport,and Birth certificate.
do i need anything else? what would be better option of sending?fax or post?
I checked all information like gender,date of birth etc.its all correct.I am worried about the medical school name.Its not been displayed anywhere in oasis or ecfmg website.I would like to know what has been registered in the medical school column.what to do?

This is wat the ECFMG says about the Name Discrepany issue:
"Changing or Verifying Your Name
If you have legally changed your name and want to request a change of name in your ECFMG record, send a completed Request to Change Applicant Biographic Information (Form 182) to ECFMG. Form 182 is available on the Publications page of the ECFMG website and from ECFMG, upon request. Form 182 requires you to provide an explanation of the reason for the name change and must be accompanied by a photocopy of the appropriate document(s), as defined on Form 182. ECFMG must be able to determine from the document(s) you submit that your name has legally changed from the name currently in your ECFMG record to the name you are requesting to appear in your record. This means that it may be necessary for you to submit more than one document to support your name change request. For the purpose of changing your name, the document(s) you provide must be unexpired (if applicable). These documents may include:
Passport (including the pages with your photograph and the expiration date)
Birth certificate
Marriage certificate
Official court order
U.S. Resident Alien Card
U.S. Naturalization Certificate
Attestations and affidavits are not acceptable as documentation to change or verify your name.
Please do not submit an original document; a photocopy of the document is sufficient.
If the name in your ECFMG record is correct but this name does not match exactly your name as listed on your medical diploma, transcript, or other credential, you must verify that the name on these documents is (or was) your name. To verify your name, send to ECFMG a photocopy of one of the documents listed above that verifies the name on your medical diploma, transcript, or other credential.
As an alternative to one of the documents listed above, ECFMG will accept for purposes of verifying your name a letter from the Dean, Vice Dean, or Registrar of your medical school that verifies that the name on your medical diploma, transcript, or other credential is (or was) your name. If you choose to submit a letter from your medical school to verify the name on your medical diploma, transcript, or other credential, the letter must be signed by the Dean, Vice Dean, or Registrar. The letter you submit must be written on your medical school's letterhead. The letter must include the following statement:
This certifies that the names [name on document] and [name in ECFMG record] belong to one and the same person.
The document(s) you send must show your name exactly as it appears on your medical diploma/transcript.
All documents submitted to change or verify your name that are not in English must be accompanied by an official English translation that meets ECFMG's translation requirements; see English Translations.
All documents submitted to change or verify your name, including translations, will become a part of your permanent ECFMG record and will not be returned to you. "
