About PGY2 Position

I am currently pursuing Prelim position in Int Medicine and am looking for PGY2 position. Now, I have a couple of issues:
1. Old Grad
2. Did MD back in home country in Ob-Gyn, but wish to do IM in US. What do You think would be "the BEST" and "most CONVINCING" answer as to why I am in IM? I ahve my own reasons but I want to know to know opinion of you Guys out there...BEST and CONVINCING answer may not necessarily be true and vice versa.
3. What are the chances of getting PGY2 position and what is the period when there are more openings and more chances of getting selected?I have not had any luck so far.
4. Another twist is I have no visa issues--am GC holder--but this has not helped me so far..in the 3 interviews I had so far. Will I be successful in getting a PGY2 position?
Plz help.Any opinion/opinions would be highly appreciated. Thank You

By yur change of specialty interviewer's impression would be that you either got bored, burnt out, or lost interest because either the schedule was very hectic, you did not like certain aspects of your specialty or you can't handle it any more because you don't have the same circumstances which favour that.Now here r a few answers for that:
If you have spent a specific time in that specialty, you can just talk about the negative aspects of that specialty at the level of policies and structure. Something that makes it sound like you enjoyed it when you started , but over the time certain changes happened in the over all structure of the specialty that kind of repelled you.
You can also mention some personal situation that compels you to change specialties like kids, relationship problems due to your hectic schedule.
If the other specialty is closely similar to the one you are leaving, you must mention from the above ones the former than the latter. If there is a dramatic difference, you shud go for the latter.
U have a good chance of getting a PGY-2 after doing a prelim IM year.IM prelim year is highly valued.Gettting a PGY2 also depends on the evaluation u get from yur current PD and Attendings.
Positions do open up round the year,and more so around in May- July of the year.U can also register with the following two websites called www.openspots.com and www.residentswap.org and loo for PGY2 positions.U can also contact various programs around yur area and look for such positions.
