I am FMG and have done post graduation in Medicine
1.During my post graduation we have to submit thesis done on clinical subject like a case control study. Should I mention that as research in the CAF.
2.In the column about Medical Education,should I mention dates of attendance since the time I joined medical school to the end of my internship period after which I was awarded medical degree.
3.Can we select both the categorical and preliminary positions in same hospital.

Yes, u can put yur PG Dissertation/Thesis as part of Research in ERAS CAF.
Yes,if yur Internship is part of yur medical education then u have to put yur dates of attendance from " start of medical school to the end of internship.
Applying to both categorical n preliminary programs at the same hospital will make programs doubt yur devotion towards a single specialty so u shud not do so.If u apply to prelim programs then programs will get to know that u have interest in other specialties ,say Neuo,Radio,etc. n not only in the categorical specialty to which u also have applied.
