A Categorical program is a 3 to 4 yrs program,e.g., in Medicine,Surgery,etc.
A Preliminary program is a one year program in Medicine,Surgery,Pediatrics.
An Advanced program is the one for which u need to complete a preliminary year program,For example, a Neurology program for which one needs to do a Medicine preliminary year first and then can three years of Neurology be done.

U need to complete yur internship in India if u have done yur MBBS from India coz' otherwise yur medical degree will not be verified.
If u have done yur MBBS from anyother country then rules governing the award of medical degree in that country will apply.

Best Wishes!

Yes, there certainly is a lack of fairness in selecting candidates for residency programs.Selecstion is not always done on the basis of one's credentials i.e., scores, experience, etc. but many a times candidates with ' not-so-good ' credentials r taken into programs on the basis of recommendations of someone they know in the program which is totally against the principle of meritocracy.

Best Wishes!

It is not easier to get into medicine preliminary year programs coz' there is a huge competition for such spots as many applicants who go into programs like radiology, neurology, etc. need to do a Medicine prelim year before they go into the respective specialties.
However, Surgical prelim prog is relatively easier to get into.
Best Wishes!
