It can never be true that every third examinee who takes the USMLE gets a 99.Because, those who say this have no authentic data backing this false premise.Yes, there has been a huge increase in the average scores of examinee starting the year 2007 when USMLEWorld came into existence and some examinees r even getting scores of around 260.We can say that nowadays there may be many examinees getting scores around the score of 90.But, there may be many as well who may not be scoring too well.
In order to get into an IM residency mean scores of matched applicants were around 228 in the year 2009.So, u shud aim for a score of over 230 or higher and pass all the steps on first attempt.
U shud also add to yur CV in the form of research, publications, observerships, etc.Try also to get excellent LORs.

Good Luck!
