Ace inhibitors are the first line of drugs used in diabetics because of its renal protective effect. One annoying side effect is cough. What else can you add relevant to USMLE?

Well, this is interesting. Supposely you should NOT use ACEI with Chronic renal Failure, BUT i looked at epocrates th eother day and it says that IF the pt is on BASELINE Cr, you can still use it in low doses. For example, if pt X have baseline Cr of 3.0 and came to the hospital and he was still 3.0 its safe to use ACEI. You shouldnt use it in Acute Renal Failure. Please correct me if im wrong. Take a look at Epocrates and let me know. Thank people...

Well I thought ACE Inhibitors give protection against Kidney disease progression such as in diabetes. Yes it may not be used in Acute renal failure but i think it is benefecial in chronic cases. Please add

I read somewhere that ACEI have "renoprotective properties" in non-diabetic nephropathies with non-nephrotic proteinuria.
It generally said that angiotensin converting enzyme inhibitors (ACEIs) or angiotensin II receptor antagonists (ARBs) have been found to slow the progression of Chronic Reanl Failure.
