On pelvic examination she has a 10-cm firm mass in her right adnexa, which has both cystic and solid components on ultrasonography. The most appropriate next step is to:

Please can someone discuss this question? I doubt the right answer.
The common management of an adnexial mass in a postmenopausal woman:
Obtain pelvic US, serum levels of tumor markers specially CA-125 and CEA.
Then we will do abdominal and pelvic ct scan to look at the tumor and to see if there is ascitis.
Now if ascitis is present (which means peritoneal seeding) then we will go for oophorectomy by laprotomy
If ascitis is not present then they can go for laprascopy!
Any comment?

obtain a carcinoembryonic antigen (CEA) tumor marker for ovarian carcnoma diagnosis, and further prognosis with therapy....as the mass has ystc and solid components..

Adnexial mass with both solid and cystic components on ultrasound may be malignant in nature hence there is need to suspect ovarian cancer.Exploratory laparotomy will be ideal and staging.
