is doing physical exam bad and history perfect can result in very low performance on ICE ?
I failed ICE because of data gathering and I am sure that I did the history perfect.

Failing one of the subcomponents of the Step-2CS exam does not mean that u need to improve that component only.It means u need to work on all the subcomponents.
How can u in the first place say that u took a perfect history ?
Generally, everyone does a focussed and a brief physical exam in the step-2CS exam and they pass comfortably.But, keep in mind that u cannot get away by merely doing an unrelated physical exam.
Did u use simple words and short sentences ? Where u louder enough and slow while talking?
Where u respectful and gentle to the patient? Did u explain each and everything to the patient ?
Did u take proper permission before doing physical exam?
Did ask questions pertinent to the chief complaint ?
I think u need to employ all these above mentioned points in yur communication before taking it second time.
Remember to practice a lot.

Yes,I mean the same.You have to work on all the subcomponents of the step-2CS.This because Step-2CS is about being wholly with the patient and yur spoken english,patient interaction skills are being judged even if u r merely asking about patient's clinical symptoms or performing a brief physical exam.
