Amyloid is a fibrillary protein which deposits in intersticial tissues resulting in the organ disfunction.Amyloid is derived from different proteins.
Common sites of involvments are:

Heart (restrictive cardiomyopathy)
Spleen
Tongue
Adrenals
Liver
Kidneys
Musculoskeletal(Carpal tunel Synd )
CNS (Alzheimer beta amyloid,Anyloid is toxis to neurons.
Medullary carcinoma of thyroid

ans. is kidney.it is imp to know that renal involvement in multiple myeloma /primary amyloidosis is actually less than other struc.

What book are you reading please ? Does this fact comes from a very recent Journal.

This is a tricky question and I don't think usmle asks about the leasts vs. most common. I am saying this is tricky because I suspect there is any evidence out there supporting the percentage of types of renal involvement in MM. I mean if kidneys primarily affected or not. But according to UpToDate you can see that in general kidney involvement is not uncommon:

"Renal disease is a common problem in multiple myeloma, although a variety of different mechanisms may be involved (43% in one series)
Myeloma cast nephropathy (myeloma kidney)
Primary amyloidosis
Light chain deposition disease
Renal tubular dysfunction" R: UPTODATE

agamamber, i do like tricky questions indeed! thanks for the question. could you please play around the answers a bit more and let us know why kidney can be the correct answer. i gave a piece of evidence that kidney is a common site to be invovled in mm. tnx in advance.
