ok first there needs to be a clarification regarding the difference between the 2 i.e observership and externship, extern is the hands on clinical experince for students from foregn med schools during the period of med training and its not for graduatess, unlike observership which is a shadowing process for fmgs

Just to add a point, some hospitals "Do" have externship programs for Foreign Grads. Just try to google the for these places or visit this site that may help:

Another point, some programs "Do" consider observership as a US Clinical exeprience.

Try contacting University of Miami/Jackson Memorial Hospital and see the Dept. of Anesthesia, also you can get an observership in Trauma ICU there (I met some anesthesiologists doing that over there). The obsies in anesthesia are not very easy to get, but if you try hard, you might get one. Good luck!

not to start this all over again , but interetsingly i came accross some open positionsn in the website where they can swap the residency positions, and there are sevERAL OPEN IN ANESTEHSIA, AMANDA IF U CAN take a look at that , it might be of some help.
