45-year-old man comes to the physician for a health maintenance examination. On the last visit, his blood pressure is 145/98 mm Hg. today its 150/98 . his family history is significant for high blood pressure, premature coronary artery disease, and diabetes mellitus. Physical examination reveals no abnormalities

Secondary hypertension is the result of a specific disease, disorder, or other condition. A hypertension within the renal arteries causes renal hypertension and it affects the entire circulatory system.

Another cause of secondary hypertension is Cushing's syndrome or aldosteronism, a metabolic disorder caused by overproduction of corticosteroid hormones by the adrenal cortex and often resulting in obesity andhigh blood pressure.

Other causes of secondary hypertension are excess hormone secretion resulting from pheochromocytoma, excess secretion of hormones by pituitary tumors, coarctation (congenital narrowing of a short section of the aorta), pregnancy, use of oral contraceptives etc. Treating the underlying condition or cause relieves secondaryhypertension.

false +ve results with drugs can increase catecholamins:
Acetaminophen ,Aminophylline,Caffeine,Clonidine,Disulfiram,Erythromycin, Insulin, Levodopa,Lithium,Methyldopa

As he is a known hypertensive.....moderate change in BP shud not set an alarm for other tests listed in options whch r used to investigate cushing's syndrome along absence of positive findings of Cushing's Syndrome......CXR is d most appropriate investigative test in this patient due to his history consistent with premature coronary artery disease to look for any features of LV dysfunction.
