First: you must do the whole Past History part, including any problems during pregnancy, delivery, puerperium, and the kid's whole PMH. Any problem from the mother is important, also alcohol, smoking and drug use history of both parents. Number of siblings, and place among them. Vaccination status, Well-being visits to the peadiatrician.
Then the questions: did he/she had proper toilet training? Was it rough or difficult? Was he/she ever proper (meaning if he/she already had proper control of the sphincters). Did this our after the birth of a younger sibling (in case he/ she is not only child.

Then you ask the parent to bring the kid for the consult, in your notes, you write Physical exam amng your Lab tests...and good luck.

Remember to always speak in lay terms (not vulgars or that you sound too vernacular you are a doctor), if the patient asks you to clarify, probably you may missing points, but do it clearly and very respectful.

If I forgot onething, please help me
