A 55-year-old Hispanic woman comes to the emergency department with symptoms of
intermittent substernal chest discomfort for the past 18 hours. Her other medical problems include hyperlipidemia, hypertension, diabetes mellitus- type 2, obesity, and hypothyroidism. She denies any history of smoking or alcohol abuse. Her medications include metformin, lisinopril, metoprolol, amlodipine, simvastatin, and levothyroxine. She is allergic to penicillin and sulfonamides. Her temperature is 37.2 C (99 F), blood pressure is 140/90 mmHg, pulse is 92/min, and respirations are 18/min.
