NBT test is a classical screening test for CGD.in this clourless dye gives blue colour if there is respiratory burst due to presence of enzyme NADPH oxidase at the cell membrane of phagocytes

you see a patient with bacterial infectionswith severe gingivitis and severe mucosal infections and with skin hypopigmentation with defect in microtubules and phagocytosis , what bug is most often the offending agent in these patints and what is the treatment

it will be staph aureus in CGD as it is catalase postive which degrades any hydrogen peroxide formed in phagolysosome

but the patient in this vignette is not suffering from CGD , whats the immunodeficiency disease he is suffering from, dear?

well you are right on the account that he will have recurrent strpt and staph infections, but he is suffering from chediak higashi syndrome, how would you go about treating this patient?

SORRY,but i did not get y is it chediak higashi

a defect in microtubule leads to failure of fusion of the lysosome and the phagosome.plus albinism is seen as an associated condition and though the mainstay remains antibiotics and surgical drainag eof absceeses , acyclovir is also used in these patients to boost immunity

what are the various precursor proteins , resulting in amyloidosis in the following diseases:
for eg :B2microglobulin in chronic hemodialysis
rheumatoid arthritis
multiple myeloma
cardiac amyloidosis
alzeihmers

for multiple myeloma ----monoclonal proliferation of B-cells...for alzeihmers----AB(beta)............
for rheumatoid arthritis---HLA-DR4,HLA-DR1.....................I GUESS i am not wrong unless you correct me.....

multiple myeloma-lambda chains
rheumatoid arthritis -serum amyloid associated protein
cardiac amyloidosis- transthyretin
amyloid precursor protein in alzeihmers

which genetic disorderis associated with the last condition

oh i badly misunderstood the question.....poor me.....

no probs..

Downs syndrome is seen in Alzeihmers

a man is transfused blood following an accident in which he has lost a lot of blood . he develops anaphylactic reaction . the relative says that there is no significant history accept a few minor respiratory infections and occasional diarrhoea. what do you think he has?

#14
Re: deep truths on immunology
kingofhearts - 11-19-08 14:53

immunocompromised??

#15
Re: deep truths on immunology
bingousmle - 11-19-08 22:42

why on earth do you say that??just because he's got chronic diarrhoea and respiratory infections?
you need to justify why has he developed anaphylactic reaction to transfused blood

Re: deep truths on immunology
kingofhearts - 11-20-08 01:03

ok it is due to allergy to plasma products....and also may be due to mismatched blood..the allergy can be subsided with chlorpheniramine maleate....

Re: deep truths on immunology

type 2 hypersensitivity

#18
Re: deep truths on immunology
bingousmle - 11-20-08 03:18

why do you say so?this man suffered an anaphylactic reaction after blood was tranfused.(hint its something to do with immunoglobulins)
but defiitely he is not immunocompromised..

Re: deep truths on immunology
kingofhearts - 11-20-08 08:39

igE mediated hypersensitivity reaction , which develops after introduction of a large shocking dose of antigen...........

Re: deep truths on immunology
bingousmle - 11-20-08 09:52

i ll not pull this any further...this man had IgA deficiency, which resulted in the recurrent infections of respiratory tract and diarrheas. but over a period of time they develop anti IgA antibodies which resulted in the anaphylactic reaction!

hmm ok

you happen to see akid 2 yrs old with recurrent bacterial infections.the production of HOCL by neutrophils is ddecreased but hydrogen peroxide and OH radicals are normally produced. what enzyme deficiency are you suspecting?

MPO deficiency

#24
Re: deep truths on immunology
bingousmle - 11-25-08 13:57

thats true. what is the metal ion in MPO

#25
Re: deep truths on immunology
pankyash - 11-26-08 08:03

not sure but i think its Cu..

myeloperoxidase contains heme as a prosthetic group, hence the metal ion is Fe
