Any one who like to get visa...the easiest way is to do electives...u may get 5 years visa as well....
wat u hav to do is to log on to yahoo.com then write in search box aamc extramural electives compendium....enter into the very first searched site (american electives organization) after opening the site , click on the right side of the site...then u wil open a new page where u can make ur own search of colleges accordng to ur own desire of location,fee, state...it wil also help in your residency by improving your cv...u can easily get job in the same department...........and u can tak your cs examination during that period as well simply by asking your concerned department head to help u out in this and they wil do the rest....

I would add that electives are for Students only, and the "extramural" ones are mostly for the American Students.

For example, a friend from a different part of the world took a month of neurosugery elective in a one of the best hospitals in the US to increase his chances in getting a fellowship once done with residency.
Another one did 2 months of Internal Medicine electives to increase his chances in getting into a good residency program.
When to do it? During med school.
How to do it? By contacting the programs/hospitals that offer it.
Who offers it? Almost all universities around the world.
Which one you should do? The one that's releavent to your pursued specialty.
