Now, u need to apply for a B1/B2 VISA to the US.If u r located in India then u need to apply for a US visa thro' the VFS website.U need to get an HDFC Bank reciept in order to register yurself on the said website.Then u can choose from the available visa interview dates on the same website.
U need to prepare the following documents in order to attend the visa interview:

1.Certificate from a Bank showing the amount in yur sponsor's or yur account.
2.Chartered Accountant Statement of yur sponsor(s) or yur's if u r sponsoring yurself.
3.TADA Affidavit.
4.Affidavit of yur sponsors that they will pay for yur expenses during yur stay in the US.

U also need to take the ECFMG Visa Letter , Scheduling Permit , Exam Date confirmation,and USMLE Scores alongwith u to the embassy.

In order to prepare for USMLE Step-2CS practice intensely the cases from the books viz. First Aid for Step-2CS and USMLE World online course for Step-2CS.Ths is more than enough for excellent Step-2CS prep.But, remember that live practice with a live practice partner is the only way to prepare for this test.U must practice all the case scenarios from the above two sources until u r fully spontaneous with all of them.
