1. What clinical training are needed for candidate with passed all USMLE exams, and clinical experience more than 10 years? Some alternatives?
2. Who can be candidate for PGY-2 and PGY-3 or PGY-4 if you passed all USMLE exams and clinical experience more than 10 years in specialty?
3.What cruicial difference in cinical training for candidates with more than ten years not practising, but with all USMLE passed, research experience and clinical practising for more than ten years before coming here?

1- Residency training, under the law "No one is allowed to practice medicein in the United States without a license" and you can't get that license without an ACGME-accridited residency training.

2- To be a candidate for a PGY-2 or higher levels you need to be done with at least 1 Year of an ACGME-accridited training in the same field or speacilaty you're seeking (aka done with PGY-1 before advancing to PGY-2).

3- As I explained on my answer no.1, everyone should go to the Match, residency in order to practice here. Both persons, with and without "ten years of clinical practice" should go through the process.

If you did a residency training in another country, not the United States, you can pass the steps and apply for a fellowship in yous pecialy's field...hard to get, but it is doable.

But as TheOne says yopu won't be able to practice in the US since you did not resdiency training here. In some exceptions you can work under supervision, but very dificult to get
