A patient develops nephritic syndrome that is characterized by electron dense "humps" on the epithelial side of the basement membrane seen by electron microscopy. A detailed history of this patient will most likely reveal that he had:

* A. History of swollen arthritic hands

the patient is suffering from acute poststreptococcal glomerulonephritis known by thr subepithelial humps.......

The electron microscopy finding described here is consistent with poststreptococcal glomerulonephritis. This type of nephritic syndrome is typically characterized by a preceding Group A beta hemolytic streptococci infection such as streptococcus pharyngitis. A history of intravenous drug abuse would be concerning for nephrotic syndromes such as focal segmental glomerulosclerosis and membranous glomerulonephritis. Swollen, arthritic hands may be the result of rheumatoid arthritis and amyloidosis. Increased urinary frequency is suggestive of diabetic renal disease. Abdominal pain after eating gluten is consistent with celiac sprue, which is associated with IgA nephropathy.
