Q1: 78 yr old male with abdominal pain & distension for 24 hr. On examination large mass in lower abdomen which is tympanic on percussion.
What is diagnosis & First best step in management

Q2: Pt with hemorrhoid c/o acute pain in perianal area ---> On Exam: tense & tender bluish purple perianal nodule ----> manage him.

Q3: Pt with celiac sprue started on gluten free diet responded very well & doing fine ---> came after 10 yr c/o abdominal pain & symptoms of malabsorption not responding to gluten free diet now.
What is diagnosis.

1----sigmoid volvulous--- npo, iv fluids , n/g tube, sigmoidoscopy and palce rectal tube and then elective sigmoid resection as chances of another volvulous are more next time

2- incise and draine ------ external haemorrhoid
3--- intestinal lymphoma
