Hip pain

A 45 year old lady presents to your emergency department with a sudden onset of left hip pain. She is having difficulty weight bearing. You order an Xray which shows a displaced fracture of the tip of the greater trochanter.

What is your next step in management?

And what type of surgery? Total hip replacement? Dynamic hip screw? Intramedullary nail? or what?

Actually if it is only the tip that is fractured then it is conservative management, with weight bearing with crutches as tolerated. Intramedullary nailing is for fractures of the shaft of the femur.

how about ORIF? Open Reduction Internal fixation... is that same as Intramedullary nailing ?

ORIF is when you open the skin to see the broken bone and usually place a plate on it. IM nailing is when you make an incision on the proximal and distal part of the bone, drill a hole in the medulla (marrow of the bone, hence the name) and place the "metal rod" into the bone and then fix it at the distal end.
