If viral load >1000 even after HAART ---> C Section
If suppresion achieved on HAART ---> patient preference

- If the viral load remains above 1,000 copies/mL, the CDC recommends delivery by C-section. When performed at 38 weeks, prior to the onset of labor, the relative risk of transmission is reduced by 50%. Perinatal ZDV, may also help reduce the risk of transmission. Also, the premature rupture of membranes (PROM) should be addressed promptly in HIV-infected mothers. Children born to mothers more than 4 hours after rupture are twice as likely to acquire HIV.
