IMGs usually go for US Clerckships in their final or Internship year.Some hospitals may need their applicants to pass TOEFL before accepting them for their Clerckship programs.U do not need to pass any of the USMLE exams before being accepted for Clerckship.

I donot think any company in India offers such a service.It cud be around 1000 USD roughly.

These premiums r meant for a year.Sometimes a hospital may also cover a student's malpractice insurance,but this is not that common.
