Loop diuretics like frusemide that will increase calicium urinary excreation ,but it needs agresive fluid hydration .This is what can I recall from hypercalcemia.

You are right.
Adequate hydration with 0.9% NaCl followed by diuretic like Frusemide.This is adequate for mild to moderate hypercalcemia ,but not for severe cases.
For severe cases--addition of calcitonin is essential.

thiazides retain calcium, too much high yeild this is asked so many times

from what i have read bisphosphonates are drugs of choice for hypercalcemia of malignancy. what is the anticancer drug which can cause hypocalcemia

what has been described above is emergent treatment,given I.V.,and bisphosphonates ar also given
AMIFOSTINE is the drug that causes serum ca to fall

I am not sure about Biphosphonates being the drug of choice.But yes it is definately used.
Anti cancer drugs causing hypocalcemia-
1. Mithramycin(plicamycin)--Was used to treat metastaic testicular cancer.Now used to treat testicular ca which has become resistant to other treatment.Very rarely used due to toxicity.
2. Cisplatin--causes hypomagnesemia which may may induce hypocalcemia.
3. 5 FU and low dose leucovorin use.

You are welcome.But I am not aware of the mechanism by which they cause hypocalcemia.Can anyone highlight on that?

fluid iv first loop diuretics thiazides contraindicated others t/t plicamycin bi phosphonates calcitonin prednisone is imp gallium nitrate

iv biphosphonates (pamidronate,sodium etidronate,zoledronic acid) are useful in malignancy asscociated hypercalcaemia...these agents inhibit osteoclast action,thereby reducing bone resorption.....
