Illinois Licensure Query

Dear friends..Pls help me with this issue..I have license from two different states in home country to practice medicine..Now that i have sent the CT form to both,one of them the first state in which i never worked, is neither filling me CT form nor issuing me a letter of good standing.They are saying that i was registered with them only for 6 months which elapsed in 2005,so they cant help me with this document......the other state is filling me CT form where i recently worked..now what should i do regarding the first state's CT form??
Can it effect my residency matching or position too?Is there any alternative by which one can overcome this problem?
Can my residency position be cancelled because of this issue?

This is what the illinois physician licensing webpage says about the CT form:
"This document must be completed by the jurisdiction of original licensure and the jurisdiction where you have most recently been practicing. This applies to individuals licensed in a U.S. jurisdiction or foreign country or province."
So, u need to send the CT form from the jurisdictions of original licensure and where you have most recently been practicing.

According to the statement above it is a must to send the CT from the place of one's original licensure.
However, why donot u talk to the illinois physician licensing board about it.
