A 4-year-old boy presents to the emergency department with fever headache for the past one day. His vaccination up-to-date. His temperature is 38.7C, pulse 98/min, blood pressure 110/60 mmHg, respirations are 16/min. He is conscious and alert. The neurological examination is non-focal, but shows neck stiffness and positive Kernig and Brudzinski signs. There is no papilledema. The rest of fysical examination is normal. The answer C is correct according to UW.

For the USMLE step 2 CK purposes, always answer Strep. pneumoniae, to be on the safe side.
Clinically, meningitis in this age always ends with Strep. pneumoniae (1st), N.meningitidis(2nd) & H. influenzae (3rd or in non-vaccinated children).
