A 23 year old male complains of a painful bump on his penis that has now 'opened'. He states that the bump first occured a week ago. The patient has a past medical history of asthma and eczema for which he is taking an albuterol inhalor occasionally and topical steroid cream occasionally. He had a pneumonia 6 months ago for which he was prescribed azithromycin, which has subsequently resolved. The patient has allergies to pencillin which gives a rash, sulfa drugs which causes a severe reaction as well as food allergies to peanuts, nuts and shellfish. On physical exam you note a painful yellow ulcer.

painful yellow ulcer....could be chancroid due to hemophilus ducreyi.....do a gram stain to see organisms....
