Left knee pain.

A 62 year old lady presents to the emergency department with a 6 month history of left knee pain. On examination she has varicous veins on both legs. The posterior aspect of her left knee is painful and hard on palpation. She cannot flex her left knee.
What is your initial diagnosis?

Follow up of the patient:
An x ray was done which showed a bony mass in the distal part of the femur (? bone tumor). The patient was refered for bone biopsy.

She has Bakers cyst. The implication in any patient who has long standing knee pain is that the erosion of the synovium may lead to formation and rupture of Baker's cyst, hence in long standing knoee pain always a search for Baker's cyst should be made.

Are you seriously going to overlook the large bone tumor and say, "hey, its probably your bakers cyst acting up again"?
