LOR is Letter of Recommendation. It is a letter addressed to a residency program director written by the faculty under whom under whom u have worked. The faculty may include any professor, lecturer, etc. In it the letter writer mentions about his/her student's attributes and strengths and explains why the student will prove an excellent candidate for the residency program. It is even better if the LOR is written on an official letterhead and signed by the letter writer.
U can ask any of the faculty for an LOR.
U can apply for a clerckship only before u graduate. It can be during Internship, final year of yur medical school, etc. Here is a website which will give an information about the schools or hospitals where u can apply for a clerckship:

Good Luck!
