25 y/o male patient came to dermatology out patient clinic complaining of pain localized to the left side of the chest increazing is severity for the last 2 days, a skin rash appeared on the same site. On examination there were grouped vesicles on an erythematous base in a dematomal distribution.The following are true except;
a. Herpes zoster is the most likely diagnosis.
b. History of prior varcieela infection is characterestic
c. Investigation for HIV infection is mandatory.
d. May be complicated by ramsy hunt syndrome.
e. Acyclovir is the treatment of choice.

bingousmle - 04-12-09 23:54 its herpes zosterr oticus. there is facial paralysis along with rash in the external auditory canal and pinna. fifth and eightth nerve involvement may result in anaestesia of face , giddiness and hearing impairment.
i would go for C.

bingousmle - 04-13-09 10:41 acyclovir is the std treatment... but famciclovir is also used.. i guess.

bingousmle - 04-15-09 10:40 the vignette very much points to a herpes zoster.. but youngdoctor, you might be correct, why dont you post a link to a reliable reference which suggests the same so we would finally zero down to an answer..

mtniharika - 09-10-09 14:25 b/c it is characteristic of herpes zoster. but according to history as it goes..there is no decription of a previous varicella inf. As HZV infection comes when immunity is low...it would probably be mandatory to check for HIV infection.
