Viruses, group A streptococci (15-20%)
Viral
Non-exudative pharyngeal erythema
Exception: Tonsillar exudate in Mononucleosis (EBV)
Vesicular OR ulcerative oral lesions
Conjunctivits in Adenovirus and Kawasaki Disease
Group A Streptococcus and other bacteria à clues are Enlarged tonsils with or without exudate , Petechiae on Soft Palate (pathognomonic) , Erythema , Tender cervical Lymphadenopathy.
à Strawberry Tongue (in Scarlet Fever)
Peritonsillar Cellulitis or Peritonsillar Abscess à Suspect Unilateral erythema of Soft Palate , Uvula deviated , Dysphagia, Odynophagia & Fever.
Diphtheria à Suspect when Gray membranous exudate covers tonsils and pharynx or Exudate bleeds easily on removal.
Kawasaki Disease à Suspect when Pharyngitis with strawberry Tongue in age
