A 57-year-old man has a 2-week history of a rash primarily on his chest but also on his back and shoulders. He received a heart transplant 6 years ago and also has gout, type 2 diabetes mellitus, chronic obstructive pulmonary disease, and mild chronic renal insufficiency. Current immunosuppressive agents are mycophenolate mofetil, tacrolimus, and prednisone, 2.5 mg orally daily. Several days before the rash appeared, he had a flare of gout and was treated with a short course of supplemental prednisone at a higher dose. The patient lives with his wife and has a small aquarium with freshwater fish. He has not used a hot tub or swimming pool since receiving the transplant.
On physical examination, he is in no acute distress. Temperature is 38.1 °C (100.6 °F); other vital signs are normal. The remainder of the examination is normal except for the skin lesions shown.
