A 20-year-old college student develops left shoulder pain after
jumping into a lake from a swinging rope. She presents holding her
arm beside her body (adducted) and avoiding any shoulder movement.
On examination, the rounded contour of the shoulder is lost and the head of the
humerus is felt under the coracoid process. Which of the following is the most
likely diagnosis?

Re: muskuloskeletal 1
conym5 - 07-15-09 19:18 Glenohumeral dislocations
may be anterior, posterior, or inferior depending on the position of
the head of the humerus in relation to the glenoid. The most common dislocation
is anterior (>90%) and is due to forceful abduction, external rotation,
or extension. There is typically flattening of the deltoid and loss of the
greater tuberosity, causing a “squared-off ” appearance of the shoulder

the rounded contour of the shoulder is lost and the head of the
humerus is felt under the coracoid process makes Anterior glenohumeral dislocation the most likely option

sackatdoc - 02-20-11 00:42 actually the arm is not in adduction but in slight abduction and externally rotated---but yes subcoarcoid is anterior dislocation only

Re: muskuloskeletal 1
babbu5508 - 03-31-11 12:45 If the patient has a dislocated shoulder, range of motion (ROM) is poor and the patient is in a lot of pain. If the shoulder is anteriorly dislocated, the arm is in slight abduction and external rotation. In patients who are thin, the prominent humeral head can be felt anteriorly and the void can be seen posteriorly in the shoulder.
