The average scores of IMGs who matched into OB and Gyn in 2009 were around 215 on both the steps 1 and 2CK.
Things in yur favour r:
1Being a US citizen
2.Research Experience.
3.First Attempt Passes
4.Ob and Gyn Experience/Observerships

Things against u:
1.14 Years Have Elapsed Since Graduation(Most Adverse Factor)
2. Scores less than 215

Doing Step-3 won't have a really drastic effect, but still it is better to take it if u feel confident of doing very well on it.
If u wanna change the line then u can go for GRE.Do u wanna do MPH or Phd?
I think side by side u shud also keep applying to the programs.Do u ahve research publications? That wud help as well.Do u ahve US LORs?Why donot u search for an Ob&Gyn externship in some US hospital?
Why do not u apply to Family Medicine?U can do further training in Ob and Gyn after completing FM residency.
Good Luck!

Ye, apply to FM and Psych again.Do u have any experience in Psych.Do u ahve any LORs in Psych.
Look again for some externship and build contacts at that program. If u r able to do so then u can get into FM or Psych.
It is the number of years elapsed since yur graduation which r causing u problems.
Best of Luck!
