mohhaider - 10-02-08 12:16 I think that vomiting is one of these factors .I think it's more associated with central causes of nystagmus .Right Sackatdoc ?

differences are again depending on history and clinical findings
peripheral has acute onset and central has chronic
peripheral a/w tinnintus or hearing loss b'cause the causes are labrynthitis, meinier's disease etc
central donot have such associations b'coz the causes are brain tumor or bleed in cerebellar region.
Now the clinical findings-
peripheral-unidirectional, i.e no matter where left or right patient is looking it will always point opposite to the side of lesion
central-multidirectional, nystagmus changes direction as person looks right or left
peripheral is suppressed with fixation i.e if you ask patient to fix his gaze at your finger, it disappears
central -is not suppressed with fixation
no FNDs in peripheral where there are in central
peripheral is never pure nystagmus. where as central is pure that is either horizontal or vertical.

mohhaider - 10-03-08 17:11 Yes , you have this only option, you ace the STEP 3 and I'm sure of that .I think it's very normal to start slowly ,then you'll be faster.

sackatdoc - 10-04-08 06:22 probably faster when i have a week left to take the exam, thats what i have done in previous two steps, i dont want to repeat that blunder.

sackatdoc - 10-04-08 13:50 for step 1, i took 5months, for step2, i took 2months, in which i used to be too relaxed in the beginning and when the date for exam would near, i used to rush with everything without giving enough time for revision, that why the results, actually i was overconfident without any substantial reason to be even confident.

sackatdoc - 10-04-08 15:13 but i'm really suffering with the kind of scores i've got, but yes with an attempt,i would have been nowhere,i have seen people suffering with attempts on USMLE.

mohhaider - 10-04-08 17:49 I think that he may get other interviews soon ,anyway who knows ,the interview he has now may work and get it especially he has one year USCE .

but to give another difference, central nystagmus is non fatiguable ,a nd usually vertical or rotational , while the peripheral nystagmus is not
