You should first identify which programs offer the observership, try usmletomd.com as it has a good list. Good luck.

Go to the website of the University of Miami, they have a link for observerships, write to them, and they would tell you what to do.
Mount Sinai, Miami beach, has an observership program called the Miniresidency program: it is in their website.
