age > 35 ; smoking ; h/o cardiovascular disease ; thromboembolic disease..
benefit of the depot medroxyprogesterone : less frequent dosing , useful for people who have c/i for estrogen..

obsulute contraindications: Deep vein thronbosis, acute liver disease, Vasular diseses, SLE, cerebrovascular accidents, hormonally dependent cancers like breast and endometrial, age >35 yrs with smoking

Relative contraindications: migrain headaches, depression, chronic hypertension, Diabetes mellitus, hyperlipidemia

Benefit of the depot medroxyprogesterone over the OCP:
higher efficacy, no need to take daily (once in 3months dose), No increased risk of deep vein thrombosis (DVT), pulmonary embolism (PE), stroke, or myocardial infarction, Decreased risk of endometrial cancer, Decreased risk of iron deficiency anemia, pelvic inflammatory disease (PID), ectopic pregnancy, and uterine fibroids, Decreased incidence and severity of sickle cell crises in women with sickle-cell disease, Decreased incidence of seizures in women with epilepsy. Additionally, unlike most other hormonal contraceptives, Depo-Provera's contraceptive effectiveness is not affected by enzyme-inducing antiepileptic drugs.

NON-CONTRACEPTIVE USE OF OCP'S:
The hormones in "the Pill" can be used to treat some medical conditions, such as polycystic ovary syndrome (PCOS), endometriosis, adenomyosis, anemia related to menstruation, and painful menstruation (dysmenorrhea). In addition, oral contraceptives are often prescribed as medication for mild or moderate acne.The pill can also induce menstruation on a regular schedule for women bothered by irregular menstrual cycles and certain disorders where there is dysfunctional uterine bleeding. In addition, the Pill provides some protection against breast growth that are not cancer, ectopic pregnancy, and vaginal dryness, and painful intercourse related to menopause.

Absolute contraindications to OCP use

Smoking greater than age 35
History of CVA or MI or multiple risk factors for coronary artery disease (older, smoker, hypertension, DM)
Moderate or severe hypertension ( baseline BP > 160/100), should also avoid in patients who have more adequately controlled hypertension as risks usually outweigh benefits
Diabetes with significant vascular complications
Complicated valvular heart disease
Thrombophilia or Thromboembolic disorder (history of DVT/PE)
Major surgery with prolonged hospitalization
Known or suspected pregnancy
Undiagnosed vaginal bleeding
Known or suspected breast cancer (or history of breast cancer)
Markedly abnormal liver function, active viral hepatitis, malignant liver tumor
Avoid in patients with symptomatic gall bladder disease current or treated
Migraine with focal neurologic symptoms (flashing lights, loss of vision, weakness, slurred speech, dizziness, cranial nerve abnormalities)
