It is a residency program. It is very competitive. It is a five year program. The five-year residency consists of one year of general surgery training followed by four years of training in orthopedic surgery.
Selection for residency training in orthopedic surgery is extremely competitive--candidates for orthopedic residencies generally graduate at the top of their medical school classes. Approximately 650 physicians complete orthopedic residency training per year in the United States. About 7 percent of current orthopedic surgery residents are women; about 20 percent are members of minority groups.
Many orthopedic surgeons elect to do further training, or "fellowships", after completing their residency training. Fellowship training in an orthopedic subspeciality is typically one year in duration (sometimes two) and sometimes has a research component involved with the clinical and operative training. Examples of orthopedic subspecialty training (fellowship) in the United States are:

Hand surgery
Shoulder and elbow surgery
Total joint reconstruction (arthroplasty)
Pediatric orthopedics
Foot and ankle surgery
Spine surgery
Musculoskeletal oncology
Surgical sports medicine
Orthopedic trauma

Good Luck!

The Mean USMLE Step-1 score of Independent Applicants(including IMGs) who matched into an Orthopedic Residency program in 2009 was 226 and the Mean USMLE Step -2CK score was 228.Their Mean number of research experiences were 3 and their Mean number of abstracts, presentations, and publications were around 7.The Mean number of work experiences they had were around 3 and the Mean number of volunteer experiences were 5 .
Other factors which can help include having a PhD degree.
So, in a nut shell, one needs to have high scores and a lot of Reseach experience and publications to get into an Ortho program.
Moreover, requiring a visa may also act as an important factor.
University based academic programs usually go for American Graduates. So, concentrating on various small programs may increase one's chances.Areas like New York,Illinois, etc. r usually considered IMG friendly.
The best way to check the IMG friendliness of the programs is to visit their webpages and look for the number of IMG residents by their names in those programs. Also, check if they sponsor visas.U can get the list of Ortho programs from FREIDA website.
Good Luck!
