Pathology 59
TheOne - 06-20-07 17:21
A 27-year-old woman presents to the emergency department complaining of wheezing and swelling of the lips. The symptoms began acutely just after eating a chicken and nut dish at a nearby Chinese restaurant. She also complains of tightness in her chest and difficulty breathing. On examination, she has a temperature of 36.8 C, a heart rate of 110/min, a blood pressure of 90/50 mm Hg, and a respiratory rate of 24/min. She is anxious and in mild respiratory distress. There is diffuse facial erythema, with swelling of the lips and tongue. Wheezing is noted bilaterally, and oxygen saturation is 92%. Which of the following cytokines is necessary for production of the antibody that mediates this response?
A. Interferon-alfa
B. Interleukin 1
C. Interleukin 2
D. Interleukin 3
E. Interleukin 4
F. Interleukin 5
G. Interleukin 6

Re: Pathology 59
TheOne - 07-03-07 16:19 The correct answer is E. This is a case of immediate (type I) hypersensitivity mediated by IgE, mast cells, and basophils. The cytokine that mediates the isotype switch to IgE in B lymphocytes is interleukin 4, produced by T-helper-2 cells. Interferon-alfa (choice A) is a cytokine that activates macrophages and would not be involved in this response. Interleukin 1 (choice B) is an endogenous pyrogen involved in the production of the acute phase reaction of systemic inflammation. Interleukin 2 (choice C) is the cytokine that induces T cell proliferation. Interleukin 3 (choice D) is a hematopoietic colony stimulating factor. Interleukin 5 (choice F) is an eosinophil stimulating factor. Interleukin 6 (choice G) is another endogenous pyrogen and a stimulator of B cells, but not an inducer of isotype switch to IgE production.

TheOne - 09-17-07 04:34 Hi guys, sorry for taking long...
Hot T Bone sTEAk is a mneumonic to easily memorize the functions or correlations of each Interlukin, lemme show you: There are small letters in the phrase..ignore them!!
Type Function Mneumonic
Interleukin 1 Endogenous pyrogen Hot
Interleukin 2 Induces T cell proliferation T
Interleukin 3 Hematopoietic colony stimulating factor Bone
Interleukin 4 Produced by T-helper-2 cells T
Interleukin 5 Eosinophil stimulating factor E
Interleukin 6 Another endogenous pyrogen A

babbu5508 - 06-21-10 02:38 E. Interleukin 4 is a key regulator in humoral and adaptive immunity...it causes stimulation of activated B-cell and T-cell proliferation, and the differentiation of CD4+ T-cells into Th2 cells.

bingousmle - 06-30-10 22:52 Interleukin 6 is another endogenous pyrogen and a stimulator of B cells, but not an inducer of isotype switch to IgE production.
