18 mon old boy brought in with fever aand urinalysis reveals that the infant is having his 3rd unrinary track infection since birth. PE wnl and no history. What is the most likely cause of the infection?

radhamesramos - 01-19-08 11:03 a. Because reflux is making urine to go retrograde and stay longer and can produce an UTI. All causes can cause UTIs but reflux is the most common

Recurrent UTI in a child of this age, most common parameter which fits his clinical discription is VUR. He is not having abdominal wall weakness or any other issues secondary to abdominal wall weakness such as constipation and he lacks features seondary to obstruction of urinary tract both of these conditions described above are seen in PRUNE-BELLY SYNDROME.
Most commonly seen abnormality is PUV-posterior urethral valve, and VUR as a secondary complication of this.
Hypospadiasis per se can cause repeated UTI'S but nothing has been mentioned in the general physical of the child about hime having hypospadiasis, hence it is also ruled out.
