Principles of HIV infected children Rx

- The ideal therapy for children has yet to be defined, but a few general principles are assumed based on limited trials, observation of the natural history of HIV in children, and extrapolation from adult data. Children tend to demonstrate very high viral loads early in life when they lack the normal immunological control of viral replication. Furthermore, children may suffer early and severe neurological and developmental injury due to HIV infection. The CD4 count is used in adults to monitor the degree of immune system suppression, but in children the CD4 decreases normally throughout the first 4 years of life.
-Therefore, therapy should be strongly considered for any child <1 year of age, any child with very high viral loads, and any child in whom adherence can be assured.

yea, and further in children PCP prop should be started in 4 weeks of age.
