I think that most patients with acute MI receive heparin and thrombolytic treatment .I guess that mural thrombosis as complication is one of it's indication.

heparin maintains t-PA induced coronary aretery patency more effectively
heparin has not been found to outweigh the harms in patients receiving streptokinase, except in pts with anterior wall MI, or pts. with complications prone to thromboembolism such as ATRIAL FIB and CCF

Unfractionated intravenous heparin and fractionated low molecular weight subcutaneous heparins are the two choices for initial anticoagulation therapy.
Augments activity of antithrombin III and prevents conversion of fibrinogen to fibrin. Does not actively lyse but is able to inhibit further thrombus formation. Prevents reaccumulation of clot after spontaneous fibrinolysis.

thanks for corroborating as how the coronary artery patency is maintained, by 2 mechanisms which are its mechanism of action of heparin, thanks YEHIA 1982
