Most of portoricans that live in New York don't even speak spanish, so make the math. And for Family medicine there are some communities in the continental U.S. where spanish is a must.

Even in us, there are a lot of areas with a lot spanish speaking population eg. california, texas, new york, nevada, etc. take classes, or get a medical spanish to english book, or find someone to teach you spanish. It makes a good impression.
