A nine year old boy complains of sore throat he has had for 2 days.In addition, he has had fever, headache, and abdominal pain. He does not have any allergies to drugs. O/E he has a temperature of 38.6 C, an erythematous pharynx and tender cervical adenopathy. A rapid screening test for group A streptococcus is performed and is positive. Which of the following would be the most appropriate antimicrobial agent ?

a-Erythromycin
b-penicillin
c-cefaclor
d-trimethoprim -sulfamethaxazole
e-azithromycin .

many strains are still sensitive to penicillin, so yes penicillin
in events of allergic reaction to penicillin,erythromycin

Correct ,it's penicillin .
Penicillin remain the drug of choice for treatment of streptococcal pharyngitis. Amoxicilin ,
macrolides and cephalosporins are acceptable alternatives .

b-penicillin should be given prophylactically to every patient of streptococcal sore throat inorder to prevent rheumatic heart disease.

rheumatic heart disease develops as an immunological reaction to the cardiac glycoproteins which resemble the M proteins of streptococci.. i doubt whether the treatment will subsequently affect the chances of RHD
