C-----------A bulge below the inguinal ligament is suggestive of lymph node enlargement.

C sounds as the best choice.

I agree with the answer "C". If it was a step-3 question, it would ask"What would the next step in management?'

Answer to next step in management is an ultrasound. it is cheap and easy. But, the answer "C" is a possiblity. However, the young boy being 15 years old, I would rule out other possiblities

as the swelling is hard and painful with only one day history an enlarged lymph node is the most appropriate choice.. i think the testicular torsion is a distractor here
