A 15 year old boy presents to you with a 1 day history of right groin swelling. He has a past history of left testicular torsion. On examination there is a hard tender approximately 2x2 cm mass in his inguinal region below appoximately the inguinal ligament level. His testicles are in place. What is your diagnosis?
A. Femoral hernia
B. Inguinal hernia
C. Enlarged lymph node
D. Infected sabaceous cyst
E. Abcess

8401glacieres - 01-24-08 10:34 C-----------A bulge below the inguinal ligament is suggestive of lymph node enlargement.

leibovici - 01-24-08 21:26 I agree with the answer "C". If it was a step-3 question, it would ask"What would the next step in management?'

leibovici - 01-24-08 21:30 Answer to next step in management is an ultrasound. it is cheap and easy. But, the answer "C" is a possiblity. However, the young boy being 15 years old, I would rule out other possiblities

bingousmle - 06-30-10 11:47 as the swelling is hard and painful with only one day history an enlarged lymph node is the most appropriate choice.. i think the testicular torsion is a distractor here
