AFter severe trauma a pt is found tohave a large plueral effusion thats turns out to be 1.5l of blood with chest tube insertion. The bleeding is moderate but steady for 4 hours later. The pt is recievin IV fluids and has recieved 2 units of blood. WHat is th enext step in mangment?
a. pleurodesis
b. repeat cxr
c. thoracotomy
d. abd ct scan
e. transfuse 2 more units of blood.

8401glacieres - 01-21-08 08:06 C---he needs to undergo thoracotomy, as the fluid drained at first attempt is 1.5 lts- a significant amount.
Indications for thoracotomy in any chest trauma case is
1)Receiving more than 1lt/1.5 lts, but for exam it's one liter.
2)Subsequent bleeding after chest tube insertion if 600ml in next 6 hrs,,,,,,,,,,,,,,,,,
low blood volume----probably from low pressure sites-------lung parnchyme and no need of thoraacotomy.
Profuse bleeding as described in the above vigneet -probably from vessels-------and needs urgent surgical care, rether than just a tube insertion,,,,,,,,,,,,,,,,,,,,,,,,,,,,,,,,,,,,,,,,,

laparotomy - 01-22-08 15:12 You should monitor his vital signs. If they are stable then concider a CT chest abdomen. To find out what is damaged and to what extent. If you see that you cannot control the bleeding then do a thoracotomy.

radhamesramos - 01-29-08 12:43 for Step III purposes, next step would be Abdominal CT if the patient's vitals are stable...and FAST is inconclusive. Thoracothomy would come next if Abdominal CT Scan is negative
