Is it a paid position or only a volunteer one?
For how much period r they offering this postion?
R they making u sign any contract?
These r the some of the question which u need to answer first.
If u need to extend yur stay in the US for yur IVs then u can file for an extension of yur stay for that purpose.
In a nutshell,if it is a volunteer research position, u can accept it on a B1/B2 Visitor visa.
However, if it a paid position with a contract, then u need to have a J1 research visa,or an H1b visa.
Good Luck!

Here is an exerpt regarding the visitor visa from the website:http://travel.state.gov/visa

"Generally, a citizen of a foreign country who wishes to enter the United States must first obtain a visa, either a nonimmigrant visa for temporary stay, or an immigrant visa for permanent residence. The visa allows a foreign citizen, to travel to the United States port-of entry and request permission of the U.S. immigration inspector to enter the U.S. The "visitor" visa is a nonimmigrant visa for persons desiring to enter the United States temporarily for business (B-1), for pleasure or medical treatment (B-2), or combination of both (B-1/B-2) purposes.

Business Visitor Visas (B-1) - For example, if the purpose for your planned travel is to consult with business associates, travel for a scientific, educational, professional or business convention, or conference on specific dates, settle an estate, or negotiate a contract, then a business visitor visa (B-1) would be the appropriate type of visa for your travel.

Pleasure, Tourism, Medical Treatment - Visitor Visas (B-2) - As examples, if the purpose of your planned travel is recreational in nature, including tourism, amusement, visits with friends or relatives, rest, medical treatment, activities of a fraternal, social, or service nature, and participation by amateurs, who will receive no remuneration, in musical, sports and similar events or contests, then a visitor visa (B-2) would be the appropriate type of visa for your travel."

U can show them the above statement from this official US government webpage.
If u seek an extension of yur stay on the basis of this research opportunity,I think there is a chance that it can result in a denial of extension, because yur visit is not primarily meant for it.Seek extension on the basis of IV invitations. That wud be a solid reason for getting an extension. But, remember to have all proofs in support of it ready.
Ye, an MD can get a J1 research visa.It is not only for PhD's.But, keep in mind that even if u get it this time then afterwards if u want to join a residency on this J1 research visa u cannot do that if there is 2yr HRR.U have to get yur J1 visa changed to J1 Clinical Visa. That is almost never possible if there is a two year Home Residency Requirement(HRR).
It is best to get an H1b visa for research and later one can transfer to a residency program which accepts an H1b transfer.
BTW, there is nothing wrong in joining a research position as a volunteer on a visitor visa as u will be recieving no REMUNERATION as is mentioned in the above exerpt.I know many people who have done so.
Good Luck!

First one shud check whether there is a 2 year home residency requirement with one's J1 research visa.If it is there then one needs to go to his/her country for two years before he/she can apply for J1 Clinical visa. The one way out is to get a waiver from yur home country's Ministry of Health.
If there is no such 2 Year HRR for yur Research j1 visa then it not an issue at all. Then it is easy to convert to J1 Clinical.

By proof in support of extension I meant the reason for extension.U shud write to them a letter regarding yur reason,if possible send them any interview date confirmation copies for which u need extension.Keep in mind that it is not essential to send IV date confirmation copies, but is only advised in order to make yur case strong.
Good Luck!
