Well ,first of all thanks God that I remember that from STEP 1.
Second ,I think that the next step is to confirm that diagnosis and surgical resection after stabilizing the patient.I don't know I still remember very little form surgery .

since i'm into step 3, which encourages me to go step wise, i'll demand the physical exam for the case first and then give d/d, which can include various things like,volvulous,colonitis, if the signs are going towards acute obstruction, then oglivie's syndrome, etc,of course diverticulitis is high on the list.

The question signs and symptoms are typically for diverticulitis including the age of the patient ,I still remember a Qbank question for STEP 1 that was very similar to this one .

first thing here to be point out, there are no signs here,only symptoms and then pain in LLQ with fever ,nausea vomitting can be n number of things, with no duration mentioned could be a tumor as well, even at age sixty!!!so be careful learning answers without giving reasons could be hazardous.

Yes ,questions here are not typical and the answers are not ideal ones .But here we are talking about the most propable diagnosis .

but my dear, there were no choices given, so the word "most", which is a superlative is incorrect in this respect, have to see what other things are to say that this is the most probable

First of all you should remeber that in theses cases you don't use surgical prosedures:
SBP, Diverticulitis and pancratitis.. this question wanted to emphesis that in LLQ pain you should remember the diverticulitis....and FEVER in this patient make it little possible to concept Tumor as the coz..The most common Dx is the infection somewhere and becoz it's in LLQ so it's very possible to be diverticulitis...the management is conservative therapy: Medical therapy+NG tube + ABs. and if resistent then surgical resection of sigmoid.

you are welcome..by the way ..I don' t have wellbeing I thik I'm goinig to have pharangitis..Damn Streptococci, May be I will be in bed for couple of days because I have a severe sore throat!

anyways the answer to question is diverticulitis and managed conservatively with nasogastric decompression, antibiotics
